Form: Request USDT or USDC Wallet Address From Customer


This form can be sent to customers if we need to make a payment or refund to a customer via USDT or USDC. Common examples are:

  • FinRev promotional bonus (e.g. "We will fund your account with $500")
  • Competition winners
  • Providing a refund via cryptocurrency
  • Paying a sales commission to a coach

Prior to sending the form, please check the client's onboarding exchange or whether the client would prefer to receive USDT or USDC, then send only the relevant form:
